MEDICAL CONCERNS/CARE/PROCEDURES

SPECIAL MEDICAL NEEDS
    If a child has a special medical need, please inform the homeroom teacher immediately.  Please check with the office to determine if this need has been placed on the emergency card.

HEALTH REGULATIONS
    Children should not be sent to school if they feel ill in the morning.  If a child is too ill to go outside for recess period, it is usually a sign that he or she is not well enough to be in school.  Since there is no supervision available for students unable to go outside at recess, students need to be well enough to go outside.
    In the event of sickness or injury in school, a child will not be sent home unless a parent or guardian is notified and comes to pick up the child.  The office staff will contact the person designated on the emergency card completed at the beginning of each year.


HEALTH,  DENTAL  AND EYE  EXAMINATIONS - MANDATORY
    Children entering Preschool, Kindergarten, and Grade 6 must have a health examination performed and signed by a physician.  A diabetes risk assessment needs to be completed for each child by their physician at the time of each physical. Health examination forms are available in the school office. 
    By May 15, 2008, all students in grades kindergarten, second and sixth must have a dental check-up completed with the appropriate form (available from the office) filed in the school office.
    Effective January 1, 2008, all students entering kindergarten must have a vision examination before October 15th of the school year.


IMMUNIZATIONS
    Immunizations required by the State of Illinois include: polio; diphtheria, tetanus, pertussis, measles; rubella; mumps; haemophilus influenza type B (HIB); Hepatitis B; and chicken pox.   Students who do not have accurate records of these immunizations on file before October 15th will be excluded from attending Holy Family School.

LEAD SCREENING
    Children through six years of age should be screened for lead poisoning prior to admission and subsequently in conjunction with physical examinations.

COMMUNICABLE DISEASE
    Please observe the following regulations issued by the Illinois Department of Public Health regarding communicable diseases:

    1. Notify the school immediately when your child has a communicable disease.
    2. Exclusion from school is required for the following, but no written release from the doctor is necessary:
a. Chicken Pox: not less than 6 days after eruption
b. German Measles: 5 days after appearance of rash (when girls have a rash, it is
    strongly urged that a physician be consulted in order that a positive diagnosis be
    made).
c.    Mumps: 9 days after the swelling is gone.
    3. All other communicable diseases, eye infections, symptoms of generalized rash, or suspicious lesions require that a child have a written release from a physician before returning to school.
    4. If your child has a sore throat, earache, discharge from the nose, skin rash, eruptions, eye infection or elevated temperature of 100 degrees he/she should be at home.

    In the event that a child contracts an illness that is contagious, it will be necessary that the school nurse check the student prior to re-admittance to classes.  This precaution is in accord with the Illinois State Code for Schools and the Illinois Department of Health.


MEDICATION PROCEDURE
When a student requires medication at school these guidelines are followed in order to assure safe administration of any medication.
1.  The Parent must complete a "Medication Authorization Form" for each medication; without this form on file, the student cannot receive the medication.
2.  A separate Medication Authorization Form is required for each medication (prescription or non-prescription) no matter how long or short a period of time the medication is to be given.
3.  Renewed medications must be accompanied by a new Medication Authorization Form.
4.  Each prescribed medication must be in a prescription bottle.
5.  Each non-prescription medication must be in a separate container, clearly labeled with:
• Child’s Name and Grade
• Name of Medication
• Dose to be Given
• Time or Frequency of Dose(s)
• Unpackaged tablets, capsules, etc. SHOULD NOT be sent to school in envelopes.
• Liquid medications (i.e. non-prescription cough syrups, liquid Tylenol, etc.) should be in containers with tight sealing lids.
• Cough Drops are considered medication and will only be dispensed from the school office.
6.  The medication will be administered from the school office by school personnel at the prescribed time.  (EXCEPTION:  With appropriate notification from the parents to the school, a child suffering with severe asthma, MAY keep his/her inhaler on said person.  The child and parent are then taking responsibility for said inhaler.)
7.  All expired medications will be disposed unless the parent picks up the medication within one week of the expiration date on the Medication Authorization Form.
8.  When a student is on long term medication therapy, it is the parent’s responsibility to assure that there is an adequate supply of the medication at school.
NOTE:  Medication Authorization Forms may be obtained at the school office during school hours.

       
ILLNESS, ACCIDENTS, OR DISEASE EXPOSURE

    If a child becomes ill or seriously injured, the parent/guardian will be notified immediately.  If the parent is not available, persons listed on the emergency card will be contacted.  This process is channeled through the office. 

TOBACCO PROHIBITION
    Holy Family School Commission on Education, in conjunction with the State of Illinois, prohibits the use of tobacco at all times within the school building.

ALCOHOL AND DRUG PREVENTION EDUCATION
    Holy Family School incorporates into its curriculum program guidelines and experiences that inform and encourage moral choices and positive self-development.  The faculty, using both internal and external resources, strives to achieve growth in self-awareness, self-esteem, and decision-making.
